- Must include answers to "yes" questions at the bottom of the page. ALL boxes must be completed and signed.
- Physical
- Must include clearance, date, and signature.
- We will NOT accept signatures from doctors of chiropractic
- UIL forms and Contact Info Forms >
- TWO forms completed online
- Name must be the same as the one you use to register for school (ie. multiple last names, hyphen between them, etc.)
- Click "continue as guest" and fill out both forms. These must be completed EVERY academic year
- Do NOT print out additional forms
**Physicals must be done for each academic year AFTER April 15th. (For the 2026-2027 school year, physicals must be dated after 4/15/26 - and physicals dated prior will be invalid).
All four forms must be completed and handed directly to the athletic training staff in order to participate. Please email sports medicine staff any questions.
